Post Acute RN

Position Summary

  • Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice and with the policies of the organization.
  • Under the supervision of the Department Leader, and with the authority from the Chief Nursing Officer, the staff Registered Nurse has the responsibility for planning, delegating and coordinating the provision of nursing care via the nursing process for the assigned patients. 
  • The RN is responsible for meeting the standards of care and practice, performing in an ethical and professional manner, and assisting in the management of staff and collaborating with ancillary departments and members of the health care team to establish and achieve patient goals and maintain high quality patient care. 
  • The RN assesses and plans for patient/family/significant other educational needs and plans, implements and evaluates knowledge gained through educational efforts. 
  • The RN must be able to demonstrate the knowledge and skills necessary to provide care, based on the physical, psycho/social, educational, safety, and related criteria for the adult, geriatric, adolescent, and child population served. 
  • The Registered Professional Nurse performs all functions in an ethical and professional manner.
     

Reports to: Unit Director
FLSA: Non-Exempt

Education: Associate's Degree Required; Bachelor's Degree Preferred
 

License: Current RN to practice in the state of Kentucky
 

Certifications: BCLS within 90 days of hire; ACLS Preferred if working in Charge RN position
 

Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
 

Essential Functions

  • The staff RN assesses, plans, delegates and coordinates the nursing care for assigned patients throughout the scheduled shift according to the plan of care and in collaboration with other members of the health care team (physicians, pharmacists, dietitians, physical therapists, respiratory therapists, social workers, etc.).
  • Assist with review and completion of MDS sections daily
    Identifies both actual and potential needs/problems through nursing assessment to facilitate an organized and prioritized plan of care.
  • Supervises and makes patient care assignments and other unit duties to other members (LPN, Nursing Assistants, UC) of the nursing team. Assignments are based on acuity and staff proficiency.
  • Recognizes and responds to emergency situations and life threatening situations appropriately.
  • Documents assessment, plan of care, interventions and educational needs of each patient, as well as, documenting the patient/family response to interventions in the patient's Electronic Medical Record.
  • Assist other personnel in the delivery of care.
  • Observe activities of nursing staff and make recommendations to the department leader or improving the quality and effectiveness of care along with reporting concerns involving patient care equipment or supplies.
  • Evaluates, communicates and documents care delivered to other members of the health care team through huddles, briefs, debriefs and bedside report, nursing care plans, progress notes/nursing notes, direct communications to team members, etc.
  • Provides patient/family education utilizing the teachback method, evaluates and recommends additional education as indicated by evaluation of patient/family learning and documents.
  • Delivers patient care activities according to standards of Care and Practice.
  • Administration of medications and treatments, i.e., initiation and maintenance of IV meds, blood and blood products, hyperalimentation, epidural analgesia, patient controlled analgesia and chemotherapy.
  • Utilization, maintenance and charging of equipment and supplies. Checks emergency equipment/drugs for proper operation and availability every shift
  • Maintains a safe patient and working environment by adhering to infection control and safety policies and procedures.
  •  Receives physician orders, documents, follows up and evaluates patients' outcomes. Assures that orders are carried out properly per policy.
  • Assist physician with bedside procedures.
  • Perform other related duties incidental to work described herein.

    Non-Essential Functions

  • Assist with AM care and bed bath.
  • Attends and participates in staff meetings, inservices, projects and committees as assigned.
  • Requesting and putting supplies away
  • Key in work orders
  • Order daily dietary supplies
  • Receptionist duties
  • Work scheduled shifts and overtime as necessary
